MEDICAL FORMS - All scouts and adult leaders must have current medical forms on file with Troop. Please submit 2 (two) copies of parts A, B1 and B2 yearly. These are for weekend campouts. 

For summer camp, and other events over 72 hours, part C is required.

PRO TIP: Get medical forms signed by a physician at yearly checkups/physicals. 

PRO TIP: Take care of part C in the spring (well before summer camp)! Must be completed annually.

Can Be Confusing! 

Please view these examples to make sure you forms are filled out correctly and your scout, our yourself, receives the care you intended.  Sometimes cell service at campgrounds are not that great and we have trouble reaching parents/guardians for permission for simple concerns. 

If a scout is required to carry emergency treatments, for example an epipen, the parent/guardian AND the scout MUST check in with the Scoutmaster (or other adult in charge) BEFORE the scout is left at camp by the parent. We want to confirm the scout actually has all necessary first aid on their person.


Emergency Contact Information: Please fill out emergency contact information for each scout and adult that will participate in Troop 105 events. This is a quick resource for leadership to contact someone in the event that you can't be reached to attend to your scout, or an adult needs assistance.
